The below information is agreed upon when signing the MAC United waiver form.  Each season, waiver forms are signed by parents and guardians on behalf of their players.  The below includes information on our Privacy Policy as well as the Refund Policy

Players moving up in a tier are required to pay an additional fee which covers the additional training provided at the higher tier.  It is always your right to accept or decline a proposed tier promotion.  Players moving down a tier will be refunded the difference.

By accepting your child's promotion to a higher tier, you are agreeing to pay the tier upgrade fee.
All members shall conduct themselves in accordance with the highest standards of morality (on and off the field), honesty, fair play and sportsmanship and will not do anything which shall be detrimental to the league, the club or team generally or which shall subject any of these entities to ridicule or contempt.

WAIVER
I hereby give my approval for the participation of my child in any and all activities under the jurisdiction of MAC United (1999).  I do hereby release, absolve, indemnify and hold harmless all organizers, coaches, supervisors, managers and officials appointed by the organizations including directors, officers and employees, from any and all responsibilities for injuries sustained by my child in connection with his/her participation in all program activities.  I authorize the Club and any of its agents to secure such medical advice and services they deem necessary for the health and safety of my child, including any person(s) transporting participants to and from the activities.  I accept responsibility for all such actions taken on my child’s behalf, including financial responsibility in excess of benefits provided for my child by any medical plan.

I give permission to MAC United Soccer Club (1999) for my child to be photographed during regular activities (practice, game, development sessions or tournaments) and give permission to "the Club" for my child’s photograph to be used to promote the program through our website, our Instagram, Twitter or Facebook page, and other advertising. Names will not be shown or tagged.

PRIVACY POLICY

MAC United Soccer Club will not use the individual information or credit information provided for any purpose other than for registration and payment for MAC United players.

As Canada's Anti-Spam laws have changed, we are now required to obtain permission to send emails containing important club information and updates.  By accepting the terms and conditions of this registration form, you are providing that permission.  Please remember, you can unsubscribe at any time.

REFUND POLICY

• After the start of the season there is a 15% administrative fee deduction.
• Full refunds are available due to medical reasons (Doctor’s note is required).
• All other refund requests are given at the MAC-United club management discretion.
• All refund requests must be emailed to info@mac-united.ca
